Air Crash Investigation Season 4 Episode 13

Air Emergency : An overloaded freight train loses control of its speed due to malfunctioning brakes. As the train enters a turn near San Bernardino, California, it derails and crashes into a row of houses, killing five people. A week later, an underground gas pipeline which had been damaged during the cleanup effort ruptures and ignites, killing two more people.
